TERMS OF SERVICE FOR CLINICAL AND DIAGNOSTIC SERVICES

Effective Date: April, 29, 2026

This Clinical Services Agreement (“Agreement”) governs the provision of medical, diagnostic, and evaluation services by ZYBS Medical Group.

ZYBS Medical Group is operated by Zybs Consulting LLC (DBA ZYBS Medical Group).

2. SCOPE OF SERVICES

ZYBS Medical Group provides clinical, diagnostic, and evaluation services, which may include but are not limited to:

  • Pulmonary Function Tests (PFT)
  • Echocardiograms (ECHO)
  • Electrocardiograms (EKG)
  • Ankle-Brachial Index (ABI)
  • Diffusing Capacity of the Lungs for Carbon Monoxide (DLCO)
  • Military Separation and Compensation & Pension (C&P) Examinations
  • Comprehensive Physical Examinations
  • Behavioral Health Assessments
  • Hearing and Audiology Exams
  • Turn-Key and Mobile Medical Clinics

Services may be delivered across multiple locations and partner facilities.

3. NATURE OF RELATIONSHIP

By receiving services from ZYBS Medical Group:

  • You acknowledge that you are engaging with licensed healthcare professionals or authorized clinical personnel
  • You understand that services may be performed on behalf of third-party organizations (e.g., government contractors, healthcare partners)
  • No physician-patient relationship is established beyond the scope of the specific evaluation or diagnostic service provided

4. NO TREATMENT OR CONTINUITY OF CARE

ZYBS Medical Group primarily provides diagnostic and evaluation services.

  • We do not provide ongoing treatment, medical management, or follow-up care unless explicitly stated
  • Results and reports are typically submitted to the requesting organization or provider
  • Patients are responsible for seeking further care with their primary healthcare provider

5. INFORMED CONSENT

By participating in any clinical service:

  • You consent to the procedures being performed
  • You understand the nature, purpose, and potential risks of the evaluation
  • You acknowledge that you may withdraw consent at any time prior to the procedure

6. PRIVACY AND HIPAA COMPLIANCE

ZYBS Medical Group is committed to protecting patient privacy.

  • We comply with applicable U.S. healthcare privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA)
  • Protected Health Information (PHI) will only be used and disclosed as permitted by law or with your authorization
  • Clinical data may be shared with authorized third parties involved in your evaluation

7. PATIENT RESPONSIBILITIES

Patients are responsible for:

  • Providing accurate and complete medical information
  • Following instructions provided by clinical staff
  • Attending scheduled appointments on time

Failure to comply may impact the quality or completion of services.

8. APPOINTMENTS AND CANCELLATIONS

  • Appointments must be scheduled in advance unless otherwise arranged
  • Late arrivals or missed appointments may require rescheduling
  • Certain services may be subject to cancellation policies depending on the contracting organization

9. LIMITATION OF LIABILITY

To the fullest extent permitted by law:

ZYBS Medical Group shall not be liable for:

  • Outcomes resulting from reliance on diagnostic reports
  • Actions taken by third-party providers or organizations
  • Delays or disruptions in service delivery

All services are provided within the scope of evaluation and reporting only.

10. THIRD-PARTY RELATIONSHIPS

ZYBS Medical Group may provide services on behalf of:

  • Government agencies
  • Contracting organizations (e.g., Veteran Evaluation Services)
  • Healthcare partners

In such cases, reports and results may be submitted directly to those entities.

11. GOVERNING LAW

This Agreement is governed by the laws of the State of Maryland, United States.

12. CHANGES TO TERMS

ZYBS Medical Group reserves the right to update or modify this Agreement at any time.
